Application Requirements US & International Students

Application for Enrollment Forms

Paperwork requirements:

  • Copy of Birth Certificate
  • Copy of high school transcript – transcribed in English
  • State Immunization records (translated in English-shot records)
  • State Health Examination
  • Valid Photo ID's
  • Guardian papers if applicable,unless other legal documents provided.
  • If divorced, legal custody documents required.
  • Completed medical authorization /emergency contact papers – Notarized
  • Completed and Signed Application
  • Enrollment Form & Fees Paid
  • Last School Attended Form
  • Withdrawal Form from previous school
Application Requirements International

Additional Paperwork requirements:

In addition to the items listed above (US REQUIREMENTS), International students must submit the following:

  • I-20 Application (pg. 19) including American physical address
  • Financial Statement Letter (pg. 20)
  • Copy of passport personal data page
  • Copy of F-1 Visa
  • Enrollment Form with Non-Refundable Enrollment Fee of $1250 paid. Only $1000 of enrollment fee is refundable ONLY if Visa is denied
  • I-20 Transfer Form if applicable (transferring I-20 from another school)

Provide Name & address to where I-20 is to be sent to

Once the original is received I-20:

Go online and complete a 901 to Immigration (cost to government  @ $250 – a one-time payment).

When you receive the I-20 you must:

  1. Go to
  2. Fill out the I-901 form
  3. Pay the fee

The following is a website that can give you a lot of information about International Students. Go to it and click on the gray tab in the middle that says International Students:

Prior to attending school - Students will not be allowed to attend class until all documents are received:

  • State Immunization Records (translated in English-shot records) See note below
  • State Health Examination
  • Withdrawal Form from previous school
  • Complete medical authorization form/emergency contact papers
  • Tuition payment in full for school year, no discounts

**Immunization Records must be on a State Immunization Form. If records need to be translated to a State form, you may do so at a local doctor’s office or clinic once you arrive in the US.

  • Enrollment fees must be paid prior to I-20 being generated. Only with written documentation from the consulate, $1000 of the Enrollment fees will be refundable if Visa is denied.
  • I-20 will be canceled if tuition not paid in full within designated time. Student will then be out of compliance and must return to their country within 10 days.
  • Tuition is paid in full for school year. Tuition and enrollment fees are Non-Refundable.
  • It is the responsibility of the parent/student, any time the student leaves the country after I-20 is issued, to have the I-20 form signed by school official once student returns.
  • I-20 will be canceled if student does not stay in compliance with attendance policies.
